If a deviation profile has been created and applied but a rule (with an assigned exception) still fails (and displays in the tree as a red "X" with a yellow background, try the following fix.
Right click on that rule in the tree and select "Remove Deviation". Right click again on that rule and select "Add as Deviation". This should save a correct exception to the deviations profile for that rule.
